What Exactly Is Basti Therapy?
Basti is one of the five core procedures of Panchakarma, and many Ayurvedic physicians consider it the most important one. In simple terms, it involves introducing herbal decoctions, medicated oils, or a combination of both into the rectum through the anal route.
Now, before anyone assumes this is just “an Ayurvedic enema,” it helps to understand why it works differently. The colon is considered the primary seat of Vata dosha in the body. Since a huge number of chronic conditions, from joint pain to anxiety to irregular digestion, trace back to aggravated Vata, treating it at its root location gives this treatment an effect that regular oral medicines often can’t match.
Types of Basti Therapy
Not all treatments in this category are the same. Depending on the condition being treated, your practitioner will choose one of these:
- Niruha (Decoction Enema): Made from herbal decoctions, honey, rock salt, and oils. Mainly used to cleanse and detoxify.
- Anuvasana (Oil Enema): Uses medicated oils as the base. Gentler, and often used to nourish and lubricate the colon.
- Uttar Basti: A specialised form used for specific urinary and reproductive concerns, done only under close medical supervision.
- Kashaya: A decoction-based variant tailored for particular Vata disorders.
Most treatment plans alternate between the decoction and oil variants over several days, following a sequence the physician decides based on the patient’s constitution and condition.

How the Basti Procedure Works
A typical session isn’t a one-off treatment. It’s usually planned as part of a course that can run anywhere from a few days to a couple of weeks, depending on what’s being addressed.
1. Purva Karma (Preparation)
Before the main procedure begins, the body is prepared through Snehana (oil massage) and Swedana (steam therapy). This softens tissues and helps the medicated liquid absorb better once administered.
2. Pradhana Karma (The Main Procedure)
The patient lies on their left side with the right knee bent. The herbal decoction or oil, warmed to body temperature, is gently introduced through the rectum using a soft catheter or a specially designed apparatus. The patient is asked to hold the position for a specific duration, which depends on the variant being used.
3. Paschat Karma (After the Procedure)
Once the retention time is over, the patient is guided to release naturally. This is usually followed by a light, easily digestible meal like khichdi, and rest is strongly advised for the remainder of the day.
Sessions are almost always done in the morning, and the entire course is monitored closely, since the sequence and combination of treatment types is adjusted daily based on how the body responds.
Who Should Avoid Basti Therapy
This treatment is powerful, but it isn’t meant for everyone. It’s generally avoided in these situations:
- During pregnancy, unless specifically advised by a qualified physician
- Active diarrhoea or dysentery
- Rectal bleeding, piles with bleeding, or recent rectal surgery
- Severe anaemia or extreme physical weakness
- Uncontrolled diabetes or heart conditions, without medical clearance
This is exactly why it should only be done under the supervision of a trained Ayurvedic practitioner, and never attempted at home based on generic instructions found online.
Basti Therapy Cost
Cost varies quite a bit depending on the city, the clinic’s experience, and whether the treatment is a single session or a full Panchakarma package. In most Ayurvedic centres in Delhi NCR, a single session can range anywhere from a few hundred rupees to a couple of thousand, while a complete supervised course, including preparatory therapies and follow-up consultations, is priced as a package rather than per session.
